New Care Management Opportunities in Consumer-Driven Health Care Landscape
Catherine M. Mullahy, RN, BS, CRRN, CCM, author of the The Case Manager’s Handbook, outlines the opportunities for case managers to become entrepreneurs in the new environment of consumer-directed health care. Those individuals who seek to move less paper and have more patient contact can increase their roles as health advocates, educators, problem solvers, and liaisons with their patients’ other medical team members. The direct-to-consumer case manager can play an essential role, especially in guiding and aiding unpaid caregivers who have the burden of care of disabled, chronically ill, and older adults. Catherine offers tips for determining if you have what it takes to be your own boss.

 

 

Continuous Glucose Monitoring: A Tool to Encourage Patient Self-Management
The importance of educating patients about continuous glucose monitoring (CGM) is described by Stacey A. O’Donnell RN, BS, CDE, diabetes educator and director of the Pump and CGM Team, at the Joslin Diabetes Center. She explains the use and limitations of CGM as a tool to help patients self-manage and prevent complications such as hypoglycemia. To aid case managers in their roles as educators, Stacey warns of the frustration and information overload some patients may feel when confronted with the amount of data CGM provides. Case managers can use this understanding to encourage and train patients in self-management and ultimately, better outcomes.
